DESCRIPTION
In this guide, NHPCO and the Center to Advance Palliative Care (CAPC) have collaborated to document the status of current hospice-based palliative care initiatives and identified the opportunities and barriers hospices face in establishing non-hospice palliative care programs across the country. The guide details the lessons hospice organizations have learned when framing and/or implementing palliative care models.

It also documents the possibilities and challenges others have observed and/or experienced firsthand.

This guide also includes a CD.
