DESCRIPTION
Increasingly hospice providers are providing hospice care within assisted living. This toolkit was developed to provide hospice providers with information that may assist them in developing successful collaborative arrangements.

Included are guidelines on:

Assisted living residences

Assessment of the resident, and

A sample Assisted Living Assessment form.
