who chaired the NCP
Revision Task Force,
with input from representatives of each of the
four major organizations in end of life — the
American Academy of Hospice and Palliative
Medicine, Hospice and Palliative Nurses Association,
National Hospice and Palliative Care
Organization, and the Center for Palliative
Care — the revised publication includes exemplars
with each domain that demonstrate how
hospice and palliative care professionals around
the country have implemented the eight NCP
domains of care in their practice settings. The
domains have not changed, but content
within the domains has been updated to reflect
changes in the field. The narrative of the document
has been updated to reflect the importance
of the NCP, particularly as the foundation
for the work of the National Quality Forum.
Most importantly, the publication features updated
references that reflect work in the field
from late 2004 to the present. The first edition
was a seminal work. This second edition takes
that content to the next level. Whether developing
a new program or assessing the current
state of your practice, this resource is musthave
for all hospice and palliative care professionals.
